Almost 5 years too late but no... It's not about literal sex changes... But a metaphor for sex. The "surgery" can be interpreted as an abortion, and the "second thoughts" and such refer to how sex changes a relationship in various ways. That's what I thought the first time I listened, too, though.

I don't think that you could ignore Brian if you tried, Dresden Dolls wouldn't exist without him. Drummers do sometimes get ignored when there's guitarists around. I guess Amanda is generally kind of the spokesperson for the band because of how vocal she is so anyone else around might kind of fade into the background a bit though outside of the performance side of the band.

Brian is an EXCELLENT drummer, but he (intentionally?) doesn't have the stage presence that Amanda does, which is why people tend to ignore him. :c
