Sister, in this context, is about a religious female of some sort (ie. nun, catholic school girl). I'm pretty sure this song is not implying incest. The lyrics, "Saints adorned the wall" and "the angels are watching/listening" - alongside the various religious imagery in the video, are huge giveaways.
